Scope Of Service

• Personal/ Respite Care
• Accompany individuals to medical appointments
• Assist with scheduling medical appointments
• Follow up on all recommendations as instructed by the physician
• Administer medication by Trained Medication employee and adequately document all physician order.  Trained medication Employee are supervised by a seasoned registered Nurse
• Develop, implement and train individuals on their program plan per interdisciplinary team
  recommendation/ approval
• Daily living assistance and home making, Grocery shopping, meal planning and preparing. All meals are geared toward healthy choice
• Kitchen coordination and hands on assistance
• Clothes selection based on the weather, laundry assistance & ironing
• Pet care assistance
• Hands on assistance with moving personal belongings
• Assistance paying bills on behalf of the individual, correspondence and general record keeping
• Personal hygiene, general appearance
• Accompany individuals to social/entertainment activities 
• Work closely with individual/ family members/ IDT regarding any specific needs or wants.
